Is there ci-CSME (center-involving clinically significant macular edema)?
73-year-old man with diabetic retinopathy and prior panretinal photocoagulation for proliferative disease that is now regressed and stable. He has also been treated with 10 intravitreal injections of an anti-VEGF agent with his last injection over 5 months ago. The best description of this OCT is non-center-involving clinically significant macular edema (nci-CSME). Certainly there are central cystic changes however based on the retinal thickness (250 microns central subfield) one can presume this would not be detectable clinically. Meanwhile the non-central changes noted on OCT temporally (400-500 microns based on the colorimetric map) are thick enough to appreciate clinically. Furthermore the retinal thickening is within 1 disc diameter of the center of the fovea and is at least one disc diameter in size. Thus the best description of this OCT is nci-CSME.
