Does all exudate warrant therapy?

We certainly all would agree that not every area of hard exudate warrants intervention. But in this case the exudation is very close to the foveal center. However, if you look carefully at the source you will appreciate that this pattern is consistent with a macroaneurysm. Many macroaneurysms resolve on their own without therapy and that is exactly what happened in this patient. With observation alone the patient had resolution of the macular exudation. The vision remained stable throughout and the patient continued her systemic therapy to treat her hypertension.
